Ontario Health Regions

Ontario Health has six regions that link us with our communities and provider partners.

Ontario Health regions work with local community and health care partners to:

  • improve patient experience
  • improve population health outcomes
  • achieve better value
  • improve front-line provider experience

We do this through:

  • implementing health system changes
  • leading the health systems within each region
  • funding health care providers
  • monitoring health care performance

Our goal is to make the health care experience better for everyone.

Our Approach

Six regions operate as the front door to our communities and health service providers. Using a population-based health planning approach, the regions help ensure that the people we serve have access to the right care and services when they need them.

North East Region

Serving communities from Parry Sound to Sault Ste. Marie to the Hudson Bay and James Bay Coasts

North West Region

Serving communities from Thunder Bay to Kenora to the Hudson Bay Coast

East Region

Serving communities from Pickering to Deep River to Hawkesbury

Central Region

Serving communities from Mississauga to Huntsville and Orangeville to Markham

Toronto Region

Serving 70 unique urban neighborhoods across Toronto

West Region

Serving communities from Waterloo to Windsor and Tobermory to Niagara Falls

Current population statistics and other data about the regions are available in the most recent Ontario Health Business Plan.

Funding

Improving the system takes planning and funding. We provide opportunities for health service providers to submit proposals and expressions of interest for planning, funding and additional resources to help improve the patient and provider experience.

Get Involved

Our patient and family advisory council (part of the Patient and Family Advisors Network) is made up of patients and caregivers in each region.

Council members have valuable first-hand experience and insights into the local health care needs, what’s currently working and how to improve the system to continue to meet future needs of our community. Diversity and geographical representation matters.
